Preparing for a visit to Dr. Hain

Be prepare answers to these questions:

  1. What is your main reason for seeing Dr. Hain (usually dizziness, migraine headaches, or hearing and dizziness problems together such as in Meniere's disease)
  2. What are the main symptoms? Do you have dizziness, headache, or hearing symptoms? When did they start, what are their triggers?
  3. What has been done to establish the diagnosis?
  4. What has been tried for treatment?
  5. Anything else we should know?

Bring in CDs of images if you have them.

If you have CDs of previous head or neck imaging, please bring them with you to the visit and ask the Medical Assistant to load them. Dr. Hain can "see" images done at Northwestern affiliates, but not ones done at other medical systems such as Northshore, U Chicago, Advocate etc.

Previous testing:

If you have had previous testing related to your inner ear, brain or neck, please bring these results with you. For hearing issues, we want to see the graphic reports. These are available if done at Northwestern, but usually not if done other than in the NM network. For example, U Chicago graphic test results are not available to NM.

Useful information:

  1. The clinic is on the 19th floor of 259 E Erie, Chicago, 60611. There is parking in several connected lots.
  2. For new visits, please arrive 30 min prior to the visit to allow for "rooming" and preliminary checks by my assistant.
  3. Dr. Hain is usually on time.
